Memorial or Tribute Gifts
Making a donation in memory of a friend or loved one - or honoring a friend, family member, colleague or Hospice of Northwest Ohio staff member with a tribute gift given in their name - is an excellent way to support the work of Hospice. The family of the person memorialized or the individual honored will be notified that you have given a gift but will not be informed of the amount.

Annual Gifts
You don’t need to honor or memorialize someone to make a gift to Hospice! Donations are welcome anytime and are vital to sustaining our "Gold Standard of Care," community education and outreach efforts. Learn more about the many programs made possible by private gifts.

Convenient Giving Options

• Credit Card or Check
    Make an online gift using a credit card OR print a gift form that can be mailed with your check.
  
  Checks should be made payable to Hospice of Northwest Ohio Foundation and mailed to:

• Stocks / Securities
  Appreciated securities (those which have increased in value) make an excellent gift. They qualify for a charitable tax deduction and may eliminate capital gains tax for you. Before making a gift of stock or securities, please contact John Lechman, President of the Hospice of Northwest Ohio Foundation, at 419-931-5187 or jlechman@hospicenwo.org.

• Monthly Gift Program
  A monthly contribution - automatically withdrawn from your bank account or charged to your credit card - is a convenient way to make regular gifts to support the work of Hospice of Northwest Ohio.  To take advantage of an electronic gift transfer please contact John Lechman at 419-931-5187 during the week, or email jlechman@hospicenwo.org informing us of the best time to contact you. 
  To sign up, you will need to provide a signed authorization form along with a voided check or voided deposit slip. Once activated, transfers will appear on your bank or credit card statement depending upon the option you choose. You may terminate this arrangement or change the monthly amount at any time by notifying the Hospice of Northwest Ohio Development Office in writing.

• Matching Gifts
  Many area employers will match the gifts their employees or retirees make to Hospice of Northwest Ohio, further enhancing the impact of their contribution. To find out your employer’s matching gift policy and process, please contact their human resources department. Typically, you would send your gift to Hospice along with your company’s matching gift form. Hospice confirms receipt of your gift by signing the form and sending it to your employer. We will send you an acknowledgment of your personal gift and will also advise you when we receive the company match.

• Tax Deductions
  The Hospice of Northwest Ohio Foundation is a 501(c)(3) non-profit organization. Our Federal Tax ID# 99-0735985. All the giving opportunities described here are tax deductible to the extent allowed by law. A receipt will be enclosed with the acknowledgement of your gift so that you may keep it for your tax records.
