The process of Advance Care Planning starts with a guided conversation with a Certified Facilitator and ends with completed Advance Directive documents. It is a proactive approach to possible future medical decisions that considers your goals and values, as well as your cultural, spiritual and religious beliefs. The process usually takes about an hour and can be repeated as often as needed as situations, goals or values change.
