| | To provide treatment and coordinate care provided to you from all sources; |
| To obtain payment from your insurance company, including Workers Compensation, for hospice services rendered; |
| To conduct daily health care operations, such as staff training, business management, bereavement mailings, etc; |
| To disclose limited information about your condition to persons inquiring about you by name while you are in the Inpatient Unit; |
| To contact you and your family as part of any fundraising activities on behalf of Hospice; |
| To obtain your treatment records from other facilities; |
| To report crimes, abuse, neglect or domestic violence, as legally required; |
| To maintain public health and safety; |
| To cooperate with law enforcement, governmental agencies and the military; |
| To respond to court orders; |
| To facilitate final arrangements. |
