NOTICE OF PRIVACY PRACTICES


Hospice of Northwest Ohio respects your privacy and safeguards your health information from unnecessary use and disclosure. The following summary of our Notice of Privacy Practices describes your rights and Hospice’s responsibilities, as guaranteed by federal law.

HOSPICE MAY USE AND DISCLOSE YOUR HEALTH INFORMATION:
   To provide treatment and coordinate care provided to you from all sources;
To obtain payment from your insurance company, including Workers Compensation, for hospice services rendered;
To conduct daily health care operations, such as staff training, business management, bereavement mailings, etc;
To disclose limited information about your condition to persons inquiring about you by name while you are in the Inpatient Unit;
To contact you and your family as part of any fundraising activities on behalf of Hospice;
To obtain your treatment records from other facilities;
To report crimes, abuse, neglect or domestic violence, as legally required;
To maintain public health and safety;
To cooperate with law enforcement, governmental agencies and the military;
To respond to court orders;
To facilitate final arrangements.

SEPARATE WRITTEN AUTHORIZATION IS REQUIRED:
   From you or your legal representative before Hospice discloses your health information for any reason other than as stated above;
   You or your legal representative may revoke an authorization in writing at any time.

YOU HAVE THESE RIGHTS REGARDING YOUR HEALTH INFORMATION:
   To receive confidential communications from Hospice personnel;
   To receive a paper copy of the Notice of Privacy Practices;
   To request restrictions on how your health information is used and disclosed;
   To inspect your record and obtain copies of your health information;
   To amend your health information;
   To receive an accounting of disclosures of your health
information made by Hospice;
   To express complaints when your privacy rights are violated.

HOSPICE IS REQUIRED TO:
   Maintain the privacy of your health information;
   Abide by the terms of the Notice of Privacy Practices;
   Provide you a copy of the Notice of Privacy Practices and all consents you sign;
   Notify you when our privacy policy is changed or updated.
