Memorial Contributions
If you would like to make a contribution to Hospice of Northwest Ohio in memory of someone who has died, you may mail it to us directly, or click here to make your contribution electronically.
When making a memorial contribution, please tell us:
  • Name of the Deceased
  • Your Name
  • Your Address
  • Your Daytime Phone
  • Name and Address of Deceased Person’s Family Member (so that we can notify them of the gift you make in memory of their loved one)

Hospice will acknowledge receipt of your gift within one business week after receiving it; the family member of the deceased, whom you have asked to be notified of your gift, will be contacted at the same time. The amount of your gift will NOT be shared with the family member.
